Things To Know

Preparing For Your First Visit

New patient paper work, including the patient information form, history form, and patient responsibility form, require 10-15 minutes to fill out before your first visit. Our staff is happy to e-mail these forms in advance of your visit date for your convenience.

Prescription Drugs and Refills

In order to expedite requests for prescription refills, please contact your pharmacy and not our office. The pharmacy will fax us a request, which provides us with a necessary paper trail to document the refill.

Medical Record Confidentiality

Your medical records are kept in the strictest of confidence. No individuals have access to your medical records without your prior written authorization.

Financial Policy

We like to be as transparent as possible with our patients as we are committed to your journey to health and wellness. In 2020, we made the decision to end contracts with all insurance carriers in order to dedicate our time to providing the best care to our patients. We will provide you everything you need (receipts of full payment as well as superbills) for you to submit to your insurance company for the reimbursement process. Every insurance company and policy is different. We cannot guarantee how much will be reimbursed with each visit, but what we can guarantee is we will help you in the process in every way we are able.

We do not provide on call service as we are not a primary care clinic. Please call 911 for help or your PCP.
